Bleeding is a condition that can stack in intensity up to 1500 times with each stack doing one pulse of damage per second. The damage dealt by bleeding is determined by the following formula:
(0.06 * Condition Damage) + (0.25 * Level) + 2
damage per stack per second,(0.06 * Condition Damage) + 22
damage per stack per second at level 80.

Traits that apply bleeding
Unrelenting Criticism (Firebrand) — Axe skills gain a chance to inflict bleeding. Symbol of Vengeance now dazes on its initial strike.
Bloodlust (Arms) — You have a chance to inflict bleeding on critical hits, and bleeds that you apply last longer.
Body Blow (Strength) — Stuns, dazes, knockbacks, pulls, pushes, and launches inflict bleeding and weakness.
Crack Shot (Discipline) — Your longbow, rifle, and harpoon gun weapon skill 1 attacks are enhanced.
Sharpshooter (Firearms) — Critical hits have a chance to cause bleeding.
Shrapnel (Explosives) — Explosions have a chance to cripple and cause bleeding on hit.
Mech Arms: Single-Edge Cutters (Mechanist) — Unlocks the Mech Command skill Rolling Smash. Your mech's attacks apply bleeding. This attack's effect may only affect a certain target once per interval.
Sharpened Edges (Skirmishing) — You and your pet have a chance to cause bleeding on critical hits.
Blood Moon (Druid) — Daze enemies when you become a celestial avatar. Inflict bleeding on enemies you immobilize or disable.
Deadly Ambush (Trickery) — Stealing also applies bleeding. Bleeding you inflict deals more damage.
Arcane Precision (Arcane) — Your critical strikes have a chance to inflict a condition based on your attunement.
Strength of Stone (Earth) — Gain condition damage based on your toughness. Inflict bleeding when you immobilize a foe. Bleeding can only occur on the same target once per interval.
Duelist's Discipline (Dueling) — Pistol attacks from you and your illusions have a chance to cause bleeding. Interrupting a foe recharges pistol skills. This trait can only reduce recharge on enemies with defiance bars once per interval.
Sharper Images (Dueling) — Illusions inflict bleeding on critical hits.
Jagged Mind (Virtuoso) — Blade attacks inflict bleeding on critical hits. A percentage of your condition damage dealt heals you.
Barbed Precision (Curses) — Critical hits have a chance to cause bleeding. Increases bleed duration.
Master of Corruption (Curses) — Corruptions gain reduced recharge time and inflict additional conditions on you.
Deathly Chill (Reaper) — Chill deals damage over time.

Oakheart Salve (Wilderness Survival) — Gain regeneration when you are inflicted with bleeding, poison, or burning. While you have regeneration, you take reduced strike damage.
