Lost in Transit is a Portal Panic achievement. After finishing the Reawakening Threat achievement, any of the characters that created the Phasic Distortion Reader should firstly speak to Mechanist Ninn, after which they can then find 5 Unusual Wreckages scattered across Southsun Cove. After finding them all, the Salvaged Device Wreckage, found at the last wreckage, needs to be given to Mechanist Ninn.
