Defeat the toxic wurm queen was a level 80 group event that occurs on the Second Level of the Tower of Nightmares. While this event was active, the path onward through the tower would be blocked; meaning players had to kill the queen to advance further.

- In the original 2013 release, the Toxic Wurm Queen's rank scaled with the number of players that are in the area when she spawns and could be either Veteran or Champion.
