Can be obtained from the Hold the Infinity Coil event![edit]
This article previously said:
"If the Crucible of Eternity dungeon is currently open, then the event chain has progressed past the point where the Commander can spawn. The defense event Hold the Infinity Coil must fail to reset the chain."
However, I can verify that this is no longer true. I believe this was fixed in in October 2018.
In the screenshot proof above, note the name of the completed event, the most recently obtained item on the right, and that the HOPE III: Prototype collection has progressed. -- Dashface
