Removal of smarty counter syntax

I've conducted a bit of a usage audit regarding smarty syntax assigned counters (which do not always initialize on a page to zero).

In many instances, the widgets didn't really need the counter logic, as the likelihood of using the template multiple times per page is low (e.g. Editing robot, or Account dungeons), so in those instances I've just removed the counter=1 check without any further changes. The remaining widgets (listed below, excluding widgets in the test category) still use counter logic, and will probably need modifying.
