The 2 Gendarran Fields Reality Rifts are reversed on the API map. When I was trying to complete this achievement, the API map kept pointing to the rift outside Lion's Arch as the one I was missing. After doing that one several times, and still needing 1 to finish, I did the rift at Guardian's Pass and got the achievement.
Though the description doesn't state it, Lion's Arch is included for this achievement, May be worth pointing out at the beginning of the article for anyone who was initially confused like myself.
