You are trapped in Tequatl's toxic undertow. Swim up to escape.

— In-game description

Watery Grave is an effect caused by being submerged in the toxic water created by Tequatl the Sunless; it deals damage over time just like  Poisoned. To remove this effect, swim to the patches of light above you so you can go into the surface, removing the effect. Several collections require players to enter the toxic waters to receive this effect.
